Dear
Parent/Guardian of New St. Mary’s Elementary Student:
- The Health Office is usually staffed by a
Registered Professional Nurse from 9:30-1:00, Monday through Friday.
- All students must have proper immunization
records before entering St. Mary’s Elementary. Records must be
submitted to the Health Office for review before the student starts
school.
- Students new to St. Mary’s Elementary and the
Lancaster School District must submit proof of a recent physical
exam by his/her doctor (done within the past year). Please have the
doctor fill out the “Health Appraisal Form” and return to the Health
Office.
- If your son/daughter has any special health
care needs and/or medical condition or history, please take time to
make the nurse aware of it.
- If your child will be participating in ANY
school sport or extracurricular activity, a current physical is
required. A physical is valid for one calendar year (i.e. a
physical dated 8/6/09 will expire on 8/6/09). Besides new entrants
and sport physicals, physicals are also required for students in
grades 2, 4 and 7.
- Any medications (prescription or
over-the-counter) that need to be taken during the school day must
have proper documentation from your physician. Please call the
Health Office if forms are needed.
- During the school year mandated screenings
will be done. Vision screening will be done on all new entrants
(this includes kindergarten students) and students in grades 1, 2,
3, 5 and 7. Hearing screening will be done on all new entrants and
students in grades 1, 3, 5 and 7. Scoliosis screening will be done
on all students in grades 5-8. If any problems or concerns are
noted, you will be notified.
- Please fill out the pink “Health Questionnaire
and Emergency Form” and return it to the Health Office. Please be
sure to include work/emergency numbers where someone can be reached
if necessary.
- Communication between home and school is very
important. I encourage you to call me with any concerns or notify
me of any change in your child’s health status.
